Lieut Calvin Shedd 7th Regt N.H. Vols having applied to me for a certificate of which to ground an application for a leave of absence. I do hereby certify that I have carefully examined this Officer and find him suffering from dibility, chronic Diarhea and inflamation of bowells, and that in consequence thereof he is my opinion unfit for duty or to travel

I further declare that it is my belief that he will not be able to resume his duties in a less period than twenty days
