Lieut Calvin Shedd 7th Regt N.H. Vols having applied for a certificate on which to ground an application for Leave of Absence I do hereby certify that I have carefully examined this Officer and fine him suffering from general debility the result of chronic Dirahea and in consequence thereof he is in my opinion unfit for duty or the travel to rejoin his regiment.

I do further declare my belief that he will not be able to resume his duty in a less period than twenty days from the date hereof
